Parent/Guardian risk warning and commitment to participation in the Double Bay Sailing Club

I, the above named (parent/guardian), consent to the membership applicant becoming a member of Double Bay Sailing Club and participating in all activities of the club. I have read and understood, and I accept, both for myself and on behalf of the applicant, the safety section of the DBSC website, the Risk Warning, Release of Liability and Conditions, the Emergency and Rescue Management Plan, and the Privacy Policy. I also agree to participate (in lieu of the applicant) as a club volunteer in on-water race management and canteen duties when rostered to do so (which will typically occur twice per season for on-water race management and once per season for canteen duties).
